LANSING — The Michigan Department of Health and Human Services HIV Care and Prevention Section has issued a Request for Proposal for HIV Care Coordination services.

Eligible applicants include government organizations, educational organizations, medical clinics and nonprofit organizations that provide comprehensive care services to populations at risk of HIV and AIDS.

MDHHS works to ensure all people living with HIV have access to comprehensive, holistic healthcare and develop skills in health literacy that promote self-sufficiency and optimal health, according to officials. HIV Care Coordination funding is intended to support efforts of applicant agencies in providing services to medically underserved people living with HIV and address health care disparities and barriers to care in order to improve health outcomes and ultimately achieve viral load suppression.

The funding priority for this application period is providing a community network of services that will seek out and engage people living with HIV who will benefit from an integrated approach to alleviate barriers to care and provide ongoing support to maintain a relationship with a patient-centered medical home.

Grants starting at $300,000 are available for organizations to provide these services. The HIV Care Coordination project period is Oct. 1 to Sept. 30, 2019. Grant applications must be submitted electronically through the MI E-Grants program by 3 p.m. May 10.
